Please bring your photo ID, insurance card. Referral ( if required), a list of current medications, and any
prior imaging or test results ( MRI, CT , EEG, etc) . If you have a completed intake form, please bring that as well.
Some insurance plans require a referral from your primary care physician. Please check with your insurance provider before scheduling.
We offer a wide range of diagnostic studies, including:
- EEG ( Electroencephalogram )
- Long-Term EEG monitoring
- NSS/EMG ( Nerve Conduction Studies & Electromyography )

At this time, our practice focuses on adult neurology. We typically see patients 18 years and older.
